These may include, but are not limited to, physical exertion, slips or falls, muscle strain, cardiovascular stress, or reactions related to existing medical conditions. I confirm that I have disclosed all relevant medical information, including past or present injuries or illnesses, and understand it is my responsibility to consult with a medical professional before beginning any new physical or therapeutic activity.
